Care planning

Useful information needs care, too.

This directory is a starting point for contacting independent organizations, not a ranking of providers.

What belongs here

We prioritize organizations with an identifiable official site, a clear local or California access pathway, and useful services for adults, children, teens, parents or families. A listing may be a clinic, school-connected service, county access route, or community support organization.

We do not sell placement, rank clinical quality, reproduce patient reviews or promise openings. The initial selection is not exhaustive. A nearby name does not guarantee a nearby appointment; program locations, residency and school eligibility matter.

What the listing tells you

Each record includes an official source, service type, age or audience, payment pathway, eligibility, location or format, language details, intake route and source-check date. Where a detail is uncertain, the listing directs you to ask. Source review is not clinical review and is not verification of an available appointment.

Planned maintenance: check urgent contacts monthly, check intake and eligibility quarterly, and recheck a listing when a correction arrives. These are future operating tasks, not claims that future checks have already occurred.

Report something that changed

Use the resource correction route. Include the organization’s name, the specific public detail and an official source if available. Do not include patient names or treatment details.

Use the form to report a public resource detail that needs correction. It does not deliver correction requests. A working production correction route is a release requirement.
